Water andmoisture also have damaging effects. Allowing any of these elementsto dry or soak on your instruments will cause undesirable stains.Other enemies include washing instruments with inappropriatesolutions such as dish or laundry soap, bleach, disinfectants, andnon-approved solutions. Cold soaking also causes damage.

 

Toproperly care for your instruments, it is important to use approvedmethods of cleaning, and to understand the causes of undesirableeffects such as staining. Surgical instruments are manufacturedfrom 300 and 400 series stainless steel. While this material rarelyrusts-- it does stain, despite its name. Stains appear as an orangeor brown discoloration. The idea is to ensure proper care to limitthese stains.

 

Causesof Corrosion (Staining, Pitting, and Marking):

Blood,pus, and other secretions contain chloride ions which lead tocorrosion, most often appearing as an orange-brown color. If lefton the instruments for any extended period of time (1-4 hours), theinstrument will mark and stain, especially if these residues areallowed to dry. The mostdamaging procedure is to allow dried-on debris to become baked-onstains in the autoclave. The temperature of the autoclave(250°-270°) will cause chemical reactions that can make the stainpermanent. Remember, an autoclave does not clean; it will onlysterilize.

 

TapWaterbr Eventap water can stain an instrument. Tap water contains a highconcentration of minerals which can be seen as a fine deposit onthe instrument surface. Rinsing with distilled water eliminatessuch deposits. Water with high mineral counts left to sit on aninstrument can cause unattractive stains. Therefore, it isimportant to dry your instruments immediately andthoroughly

 

Cleansers

Thecleansers and cleaning agents you use could also be a cause ofcorrosion. Strong substances, as well as those containing achemical make-up of acid or alkaline-based solutions can lead topitting and staining. Wash instruments with a neutral pH soap(between 7pH - 8pH) for optimal results. Anything higher may damagethe instrument and is not necessary. Do not use Betadine® Solution,dish soap, laundry soap, or surgeons hand scrub. These productswill cause spotting and corrosion. Using an instrument cleaningbrush is a good idea, especially for jaw serrations, teeth, andhinged areas.

 

CleaningAfter Surgery:

Thewashing process should begin within 10 minutes after surgery, evenif sterilization will take place much later. Washing instrumentswithin a few minutes of surgery is your best defense againstcorrosion, pitting, and staining.

 

Use onlyapproved solutions. Non-approved solutions are any that do notspecifically state on the label that uses include surgicalinstruments, stainless steel, and sterilization.

 

Approvedsolutions are specially designed for surgical instruments and thesterilization cycle. Their product labels will state thisuse.

 

Sterilization:br Allsurgical instruments must be sterilized prior to surgery to preventinfection. But even sterilization can leave contaminants behind ifinstruments are not properly cleaned. In fact,ultrasonic cleaning is 16 times more efficient than hand-cleaning.Place instruments in the ultrasonic unit for 10-15 minutes and usea neutral pH solution.

 

Here area few more tips for ultrasonic cleaning:

- Beforeplacing into the ultrasonic unit, clean instruments of all visibledebris by washing them in an approved instrument cleaningsolution.

- Don''tmix dissimilar metals (such as aluminum and stainless) in the samecycle.

- Makesure instruments have plenty of room. Don''t overload yourultrasonic cleaner.

- Aswith all types of cleaning, open all instruments so ratchets andbox locks are fully exposed to the cleaning process.

- Uponcompletion of the cycle, remove instruments immediately, and rinsethem.

- Drythoroughly with a towel, ensuring that no moisture is left on theinstruments.

 

The useof water in an ultrasonic cleaner is not recommended. A neutral pHultrasonic cleaner solution, when properly mixed, effectivelyreduces the surface tension of the solution and increases theultrasonic cavitation process.

 

Thesolution should be changed at least daily, or sooner if thesolution appears dirty or murky.
